Programs
American Red Cross
Community Transportation Program
This program provides transportation services to low income, senior and disabled individuals unable to access life enhancing medical appointments and other wellness activities on their own. This service is able to pick citizens up at their homes, drive them to receive medical services, and drive them home again.
Amethyst, Inc.
Bridge to Care
Amethyst’s Bridge to Care program strives to reduce healthcare barriers associated with access and to provide linkage to services. This program targets individuals and families with histories of addiction and mental illness, unstable housing, and lack of preventive healthcare and/or care for chronic conditions.
Catholic Social Services
Senior Supportive Services
The Supportive Services Program increases the ability of persons 60 and older to remain safe and independent, living in their own home, by providing assessment, information & referral, linkage to health and community services as well as brief or ongoing intervention from licensed social workers.
Central Ohio Diabetes Association, Inc.
Prevention & Detection Program
This program screens adults for undiagnosed and uncontrolled diabetes by testing their blood glucose level. It also evaluates weight status for diabetes risk. Screenings are provided in un/under-insured, minority and other high-risk communities where individuals do not have regular health care.
Clintonville-Beechwold Community Resources Center
Senior Supportive Services
The Senior Supportive Services program provides individual assessment and on-going case management services particularly as it relates to accessing healthcare needs and keeping seniors in their homes. The Center works with the most vulnerable seniors who live in poverty and live with disabilities due to physical or mental health impairment.
AIDS Resource Center Ohio
Integrated Referral & Case Management Program
The intent of this program is to ensure that all individuals who test positive for HIV at AIDS Resource Center Ohio will be linked to the appropriate medical, financial and support services successfully. HIV testing specialists will provide referral information packets to those who are newly diagnosed and will offer assistance to establish initial linkages to infectious disease specialists, support groups, drug assistance programs, mental health services, HIV case management, and other supportive services.
LifeCare Alliance
Columbus Cancer Clinic - Home Care Support Transportation
The LifeCare Alliance’s Columbus Cancer Clinic Home Care Support transportation program provides low-income, under insured or uninsured individuals living with active cancer with transportation to and from cancer related medical appointments via taxi, cab, or shuttle service. include a lack of transportation.
Columbus Cancer Clinic -Screening & Linkage
LifeCare Alliance’s Columbus Cancer Clinic screening program links patients with abnormal findings through a screening exam for follow-up appointment with physicians, Neighborhood Clinics, Access Health Columbus or BCCP to help secure follow-up treatment.
Visiting Nurses
LifeCare Alliance's Visiting Nurse Association nurses assess related health care needs of patients, with a focus on low-income seniors and chronically ill individuals who are at high risk for health issues, in addition to their nursing assessment for skilled care.
Health Care Transportation
Maryhaven's Healthcare Transportation Program arranges for or directly provides 10,650 rides a year for persons who otherwise could not access needed medical and behavioral health services. Maryhaven arranges for transportation to and from Maryhaven and to scheduled physical and mental health appointments for patients needing care.
Linking Women & Children to Medical Homes
This program provides residential treatment to women recovering from addiction to alcohol and drugs. This 4-6 month program particularly targets pregnant women and women with young children who may reside at the program with their mothers.
Access to Health Care
This program helps people to identify their health needs and then links them to services to meet these needs, and provides them with a Continuing Care Plan to continue to receive needed services. Many people encounter barriers to accessing health and wellness services including lack of awareness of their health conditions, lack of knowledge about services, lack of money and insurance, and others.

Accessing Mental Health Services
This program is designed to link people with mental health issues to appropriate healthcare services, resulting in the improved health of the participants.
Physicians CareConnection
Physicians Free Clinic - Voluntary Care Network
The Physicians Free Clinic/Voluntary Care Network is the largest coordinator of voluntary health care services in Franklin County with over 1000 primary/specialty care doctors and 12 hospitals participating. This network connects low-income, uninsured residents with medical homes and improves access to specialty care.
Senior Independence (Ohio Presbyterian Retirement Services)
Transportation
This transportation program helps seniors maintain their independence by providing access to essential medical care and services. Unique features of the program include: door-to-door service, wheelchair-accessible vehicles, group trips, and advanced scheduling.
The Breathing Association
Accessing Lung Health & Wellness
The Accessing Lung Health and Wellness service provides 5500 linkages to help 4000 low income and minority individuals in Franklin County overcome barriers to accessing health and wellness services. Agency programs work together to establish linkages that help clients attain improved health outcomes and increased satisfaction with health and wellness services.
Refugee Health Care Access
This program’s intent is to remove barriers to healthcare for refugees, increase their knowledge of the American healthcare system, and assure they are receiving treatment for their many health problems. Services include comprehensive health assessments; developing individual health care plans; linkage with community health resources/benefits and providing education workshops.
